To receive consideration for a Superintendent position listed on the IASB website, a Personal Information Sheet must be completed in full. You may obtain the sheet from the IASB office or download the Adobe PDF document here. A separate sheet must be completed for each position sought.

The online version of the Personal Information Sheet may be completed one of two ways:

1. Print and Complete: Simply open the document using Adobe Acrobat Reader and print a copy of it. Then complete the form, sign it and mail it to the IASB office indicated for the vacant position.
   or
   
2. Fill out and Print: Open the document using Adobe Acrobat Reader and fill it out online. Use the tab key to move from field to field or click in any of the fields to be completed, then type your information. When the sheet is filled out, print it, sign it and mail it to the IASB office indicated for the vacant position.

Please do not e-mail the online Personal Information Sheet; your signature is required.

NOTE: Unless you are using a full version of Adobe Acrobat, you will not be able to save a completed form on your hard drive. The Acrobat Reader will save only the original blank form. To retain a copy for your files without the full version of Acrobat, you will need to make a photo copy.
